SECTION 39-11-5



   § 39-11-5  Application for certificate

– Fee. –

Application for a certificate shall be in writing and shall contain such

information as the administrator may require. All applications for a

certificate shall be in the form prescribed by the administrator and shall be

accompanied by a filing fee of twenty dollars ($20.00).
