Applications, form, contents, fees.
336.040. Every person who desires to obtain a license to practice
optometry shall apply to the board, in writing, on forms prepared and
furnished by the board. The application shall state the required
qualifications of the applicant, that the representations are made under oath
or affirmation and are true and correct to the applicant's best knowledge,
subject to the penalties of making a false affidavit or declaration and shall
be accompanied by the examination fee.

336.040. Every person who desires to obtain a certificate of
registration shall apply therefor to the state board of
optometry, in writing, on blanks prepared and furnished by the
state board of optometry. Each application shall contain proof
of the particular qualifications required of the applicant, shall
contain a statement that it is made under oath or affirmation and
that its representations are true and correct to the best
knowledge and belief of the person signing same, subject to the
penalties of making a false affidavit or declaration and shall be
accompanied by the examination fee.
